Your shingles rash
is gone. The pain
never left.
Dr. Demetrio Aguila is one of a small number of surgeons in the United States who performs peripheral nerve surgery for postherpetic neuralgia — the chronic pain that persists after shingles. Patients travel from all 50 states because surgery works when nothing else has.
If you've been told
there's nothing
more to be done —
Postherpetic neuralgia (PHN) is the chronic nerve pain that follows a shingles infection. The rash heals. The pain does not. For many patients it persists for months, years, or decades — and most are told it's simply something they have to live with.
Dr. Aguila disagrees with that conclusion.
You may be a candidate for peripheral nerve surgery if:
Surgery for PHN is not right for everyone — but for patients who meet certain criteria and have exhausted other options, it can offer relief that medications alone cannot provide.
Out-of-state and international patients are the standard at Total Pain Solutions. We coordinate care for patients traveling to Omaha.

A surgeon who
pushes past "nothing
can be done."
Dr. Demetrio Aguila is a pain-focused peripheral nerve surgeon and the founder of Total Pain Solutions in Omaha, Nebraska. He is one of a small number of surgeons in the United States who specializes in surgical treatment for chronic pain — conditions that most patients have been told cannot be resolved.
Dr. Aguila trained under Dr. A. Lee Dellon, the physician who pioneered the field of peripheral nerve surgery for chronic pain. He is a diplomate of the American Board of Plastic Surgery and the American Board of Otolaryngology, a Fellow of the American College of Surgeons (FACS), and a member of the American Society of Peripheral Nerve (ASPN).
Patients travel from all 50 states and 32 countries to his practice in Omaha — not because it is close, but because the surgical treatment he provides is available from very few physicians anywhere in the world.
Before founding Total Pain Solutions, Dr. Aguila served as a flight surgeon in the United States Air Force — an experience that shaped both his diagnostic precision and his commitment to his patients.
He believes
you deserve
a real answer.
Most of Dr. Aguila's patients arrive after years of suffering and a string of physicians who told them their pain was simply something to manage — not resolve. He disagrees with that conclusion.
His surgical approach targets the nerve pathology directly, addressing the underlying cause rather than managing symptoms indefinitely. For the right patient, surgery can provide relief that decades of medication could not.
Every patient who reaches Total Pain Solutions has typically already fought hard to get here. Dr. Aguila takes that seriously.

despite the distance.
They come because of what's here.
Peripheral nerve surgery for PHN is performed by a very small number of surgeons in the United States. If you are suffering from shingles pain and have not found relief, geography should not be the reason you don't pursue this option.
We routinely coordinate care for out-of-state and international patients. A virtual consultation can often be arranged before you travel.
